  When using the term “personal data” in our Privacy Policy, we mean information that relates to you and allows us to identify you, either directly or in combination with other information that we may hold. Your personal data may include for example your name and contact details.  

What we collect

We may collect and process the following information:

Your full name, date of birth and your contact details such as email address, postal address and phone number.

Personal relevant medical and health history.

Information about your transaction, including your payment card details and relevant forms of Identification

The communications you exchange with us (for example, your emails, letters, calls or your messages on our online chat service)

  How we get the personal information

Most of the personal information we process is provided to us directly by you for one of the following reasons: - To be able to diagnose and provide healthcare treatment to you.

-To provide you with information regarding available services and appointments  

We also receive personal information indirectly, from the following sources in the following scenarios:

      Log Data – like many site operators, we collect information that your browser sends whenever you visit our Site (“Log Data”). This Log Data may include information such as your computer’s Internet Protocol (“IP”) address, browser type, browser version, the pages of our Site that you visit, the time and date of your visit, the time spent on those pages and other statistics. In addition, we may use third party services such as Google Analytics that collect, monitor and analyse you actions on the website. ·      

Cookies – these are files with small amount of data, which may include an anonymous unique identifier. Cookies are sent to your browser from a web site and stored on your computer’s hard drive. We may share your personal information with relevant medical and healthcare professionals to help provide or to ensure you receive ongoing diagnosis and healthcare treatment. This is only done with your consent, however.

Under the UK General Data Protection Regulation (UK GDPR), the lawful bases we rely on for processing this information are: (a) Your consent.

How we use your personal data

We may use your personal data for the following purposes:

-       To enable our business to respond to enquiries, referrals and contacts about the quotation or provision of professional hearing services [treatments for the removal of excess ear wax] as advertised in print, signage, online at www.clearearstudio.co.uk

 and through associated social media platforms. Enquiries, referrals and contacts include those received through phone calls, emails and online enquires; or in response to e-newsletters/online, print advertising and social media.

-       To advise or remind you about your appointments with us and to perform pre-appointment triage. -       To maintain a client record card detailing your health history and treatment. A copy of this record is available on request.

-       To keep in touch with you during the planning and delivery stages of the hearing treatments and services you have contracted us to provide. 

-       We may occasionally contact you by email or SMS message to follow up about the service you have received or to inform you of similar services we offer that may be relevant to you. You will be invited to opt-in to our mailing list. You can unsubscribe at any time and we never share our mailing list contact details with third parties.       

How we store your personal information

Your information is securely stored and retained for 10 years after the last clinic visit made. In the case of children and young people, information is securely stored and retained until the patient’s 25th birthday or 26th if young person was 17 at conclusion of treatment.

Your personal information is stored to be able to provide ongoing diagnosis and healthcare treatment or in the event of complaint, adverse event or impending litigation. In the event of death, they are persona information is stored for a period of 8 years.

At the relevant time, all personal information is disposed of securely, ensuring that patient confidentiality is protected.

Examples include cross-cut shredding, incineration or utilising a commercial company holding the necessary accreditations.

For computer-held records they are permanently deleted from relevant databases and from the hard-drive recycling bin.  

Your data protection rights

Under data protection law, you have rights including: ·      

Your right of access – You have the right to ask us for copies of your personal information.     

Your right to rectification – You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete. ·      

Your right to erasure – You have the right to ask us to erase your personal information in certain circumstances. ·      

Your right to restriction of processing – You have the right to ask us to restrict the processing of your personal information in certain circumstances. ·      

Your right to object to processing – You have the the right to object to the processing of your personal information in certain circumstances. ·      

Your right to data portability – You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.

– Your postal address
– A telephone number where you can be reached
– A copy of your passport or driving licence
